Please fill in one congress registration form per delegate and accompanying person.
You will receive a personalized badge when collecting your registration documents. During the Congress, this badge must be clearly visible at all times and will grant you access to the Scientific Sessions and the Commercial Exhibition.

For group registrations (more than 10 persons), we kindly ask you to contact MONDIAL CONGRESS & EVENTS to arrange details.

REGISTRATION FEES (IN EURO)

Please read all REGISTRATION INSTRUCTIONS below.

* Please note: Members and residents have to provide a membership/resident document at the registration desk when registering. If the delegate who has registered as a member/resident does not provide a valid membership pass/confirmation of residency, non-member/regular member/non-member fees will be charged.

DAY TICKET REGISTRATION FEE INCLUDES:

Admission to all scientific sessions
Admission to poster- and commercial exhibition
Conference materials (congress bag, final programme, abstract proceeding, badge)
Coffee Breaks on that day
Lunch Break on that day
Opening Ceremony & Welcome Reception or Closing Ceremony (only applicable for day tickets valid on day of ceremony)
Certificate of attendance

ACCOMPANYING PERSONS REGISTRATION FEE INCLUDES:

Admission to general areas of the Congress Center (not to Scientific Sessions!)
Closing Ceremony
Ticket for Public Transportation (metro, bus, tram) within Budapest during the congress

Please note that registrations can only be accepted and confirmed upon receipt of the completed registration form (mailed, faxed or filled in via the website www.eurotrauma2008.org) and receipt of full payment. For information on payment, please refer to "TERMS OF PAYMENT".
The date of the remittance order (as stamped on the slip) will be decisive for early-bird and regular registration (i.e.: for early-bird registration, the remittance has to be ordered by the date from February 14, 2008 to February 15, 2008).
As soon as the Organising Secretariat has received your payment, a letter of confirmation will be mailed. Confirmations will be e-mailed if an e-mail address is provided on the registration form. This option is highly recommended as it helps avoid time consuming correspondence. Otherwise, confirmations will be faxed or land-mailed.

CANCELLATION POLICY – REGISTRATION

Refunds - less EUR 40 administration fee - will be granted to delegates unable to attend, provided written notice is received by MONDIAL CONGRESS & EVENTS by February 15, 2008. After this date, no refunds are available. Amounts due will be reimbursed after the Congress.

Please present your letter of confirmation at the registration desk upon arrival.
